पहचान-पत्र (Identity card) जन्म प्रमाण-पत्र (birth certificate) पते का प्रमाण-पत्र या सबूत (Address proof) फोटोग्राफ (photograph)
ध्यान रखें कि इन सभी की आप स्कैन कॉपी तैयार रखें. वरना यह सब होगा भी तो आपको फॉर्म अपलोड करने में बहुत परेशानी होगी.
आइए जानते हैं आपको पहचान-पत्र (Identity card) के रूप में किन दस्तावेजों की जरूरत होगी. Unique Identification Authority of India Government of India की ओर से जारी किया गया Aadhar Card पासपोर्ट passport, मतदाता पहचान पत्र या वोटर आईडी कार्ड voter id card, ड्राइविंग लाइसेंस driving license फोटो के साथ राशन कार्ड या फिर आर्म्स लाइसेंस, या केंद्र-राज्य सरकार की ओर से या पीएसयू द्वारा जारी किया गया फोटो पहचान-पत्र, पेंशन कार्ड जिसमें तस्वीर हो, केंद्र सरकार का कोई स्वास्थ्य बीमा कार्ड, या सांसद/विधायक के हस्ताक्षर किया हुआ पहचान-पत्र भी लगाया जा सकता है.
इसके अतिरिक्त पते के लिए आपको उसका प्रमाण-पत्र भी देना होगा. इसे पते के सबूत के तौर पर देखा जा सकता है. इसमें आपको.
